Flu shots available by signup only this year

This year, because of COVID-19 restrictions, parishioners will need to sign up ahead of time to receive a flu shot. 

Flu shots will be offered by signup only on Friday, October 2 from noon to 2:30 p.m. and after both weekend Masses. There are two ways to sign up:

1. For those signing up to attend weekly Mass, select “Yes, I would like a flu shot.” 
2. For those not attending weekly Mass, sign up here.

Signup for both options opens at 5 p.m. on Sunday, September 27 and closes at noon on Thursday, October 1. Please note that all parishioners arriving for flu shots who did not attend Mass must pass the health screening used for Mass attendance. The Health Ministry will conduct health screenings and usher parishioners to flu shot stations.

What to bring

• A completed immunization screening form (Screening forms will also be available onsite.)
• An original or a photocopy of your insurance card. Your original card can also be photocopied or photographed on site.
